President & Executive Director

Nate West was born and raised in Phoenix, Arizona, and is firmly rooted in the local community. A proven business leader, Nate owns and operates multiple local businesses, and he takes pride in providing valuable services to his community while creating meaningful employment opportunities.

He holds both a Finance degree and an MBA from Arizona State University. As a Marine Corps Officer, Nate deployed to the Middle East, leading over a hundred Marines and honing his leadership and resilience—qualities he tries to bring to every aspect of his work today. Heritage Home Care proudly operates as a veteran-owned small business under his leadership.

Nate’s decision to enter the caregiving industry was deeply personal. Having witnessed the immense effort and dedication his mother put into caring for her own mother, he saw the emotional and physical challenges families face when tasked with providing care. He believes everyone deserves the choice to stay in the comfort of their own home, and he is passionate about making that a reality through Heritage Home Care.

Outside of work, Nate volunteers with the Salvation Army, Arizona Friends of Foster Children Foundation, and Big Brothers Big Sisters. A sports enthusiast, he supports all the local Phoenix teams and loves spending time with his wife, Becky, and their two dogs.

Justin Score

Vice President & Director of Business Development

Justin Score serves as the Vice President of Heritage Home Care, leveraging over 15 years of experience in the medical industry. He holds a degree in Health Sciences from Arizona State University, which forms the foundation of his expertise in healthcare management and senior care.

Justin’s passion for senior care stems from personal experiences with his aunt and uncle, both of whom faced the challenges of dementia. Their journey inspired his commitment to providing compassionate, high-quality care to every client at Heritage Home Care. Justin is dedicated to fostering trust and ensuring each client receives personalized attention tailored to their needs.

Outside of work, Justin enjoys spending time with his family, traveling to new destinations, and playing golf. He is also a proud dog owner to Fitzgerald and cherishes moments with his son, Mike. Justin’s dedication to improving the lives of seniors guides his leadership and vision for Heritage Home Care.

Emily Tao

Field Support Supervisor

Emily Tao brings a wealth of experience in senior care, with a deep-rooted passion for person-centered support. Starting her career as a certified caregiver and house manager in assisted living group homes, Emily developed hands-on expertise in providing compassionate, individualized care. Recognizing the opportunity to extend her impact, she transitioned into in-home care, where she could help individuals remain comfortable and supported in their own homes.

Over the years, Emily has honed her skills in recruiting, care management, and operations, enabling her to develop a comprehensive understanding of the senior care continuum. Now, as Director of Community Relations, Emily focuses on building meaningful partnerships with organizations that share her philosophy of empowering seniors through education, resources, and personalized care solutions.

Emily takes pride in serving as a trusted advocate for seniors and their families. She is dedicated to connecting them with valuable resources while fostering relationships that enhance the well-being of those in need.

The youngest of seven children, Emily values family deeply and enjoys spending her free time with loved ones and her two dogs, Bailey and One-Eyed Willie. Her experience, knowledge, and unwavering commitment to supportive care make her an invaluable asset to the communities she serves.

Kelly Shepard

Director of Operations

Kelly is the Director of Operations at Heritage Home Care, where she oversees daily operations to ensure all processes run smoothly and efficiently. With nearly 20 years of experience in the healthcare space, Kelly brings a wealth of knowledge in operations management, process improvement, and team leadership. She plays a key role in driving the company’s growth and maintaining high service standards. Her expertise includes streamlining workflows, enhancing productivity, and implementing technology solutions to optimize performance across departments.

Driven by a passion for creating systems that support both the team and the clients, Kelly thrives on finding innovative solutions to operational challenges. She believes that effective operations are the backbone of any successful company, and that a well-organized team is essential for delivering exceptional client experiences.

Outside of work, Kelly enjoys reading, rooting for her favorite sports teams, and volunteering in her children’s schools. She is married with two kids, two dogs, a cat, and two tortoises—her home is always full of energy and activity! Kelly is excited to be part of the Heritage Home Care team and is committed to helping the company scale its operations while maintaining a focus on quality and client satisfaction.

Cendy Mendoza

Field Support Supervisor

Cendy has been an integral part of Heritage Home Care since 2018, serving as a Field Support Supervisor. With a background as a Nursing Assistant and Patient Care Technician, she brings over six years of hands-on experience in patient care. In her role, Cendy works closely with field staff to ensure the highest standards of care and support for clients, while also managing day-to-day operations to enhance service delivery.

Cendy’s passion for caregiving stems from her desire to make a difference in people’s lives. Her approach is built on compassion and empathy, ensuring that every client receives not only top-tier care but also the emotional support they need. She believes that communication and teamwork are essential to providing the best possible outcomes for both clients and staff.

Outside of work, Cendy enjoys hiking, traveling, cooking and spending quality time with her son and dogs. She also takes part in a choir that helps shed light on people with Alzheimer’s and dementia. Cendy is proud to be a part of the Heritage Home Care team and remains dedicated to improving the lives of those in her care.
